Learn how to use AzraielForms to create powerful forms for your community

Getting Started

Welcome to AzraielForms! Here's how to get started in just a few steps:

  1. Create an account - Sign up with your email and password
  2. Go to Dashboard - After logging in, you'll see your dashboard
  3. Create a new form - Click "New Form" and choose a template or start blank
  4. Add fields - Use the drag-and-drop builder to add your questions
  5. Configure settings - Add your Discord webhook, password protection, etc.
  6. Publish and share - Toggle publish and share the link!

Creating Forms

AzraielForms offers multiple field types to build comprehensive forms:

Pro tip: Use templates to quickly create common form types like vetting applications, task assignments, and session feedback forms.

Discord Webhook Setup

Get instant notifications in your Discord server when someone submits a form. Here's how to set it up:

Step 1: Create a Webhook in Discord

  1. Open your Discord server and go to Server Settings
  2. Click on Integrations in the left sidebar
  3. Click Webhooks then New Webhook
  4. Give your webhook a name (e.g., "Form Submissions")
  5. Select the channel where you want submissions to be posted
  6. Click Copy Webhook URL

Step 2: Add Webhook to Your Form

  1. Open your form in the AzraielForms editor
  2. Go to the Settings tab
  3. Find the Discord Webhook section
  4. Paste your webhook URL in the input field
  5. Save your form

Important: Keep your webhook URL private! Anyone with this URL can send messages to your Discord channel.

Password Protection

Protect your forms with a password so only authorized users can access them:

  1. Open your form in the editor
  2. Go to the Settings tab
  3. Find the Password Lock section
  4. Toggle the switch to enable password protection
  5. Enter your desired password
  6. Save your form

How it works: When someone opens your form link, they'll see a password gate. They must enter the correct password to view and submit the form. The password is remembered for their browser session.

File Uploads

Allow users to upload files with their submissions:

Supported File Types

  • Images: PNG, JPG, GIF, WebP
  • Documents: PDF, DOC, DOCX, TXT
  • Other common file types

Limits

  • Maximum file size: 5MB per file
  • Files are securely stored and accessible via URL

Tip: File upload URLs are included in Discord webhook notifications, so you can easily view submitted files.

Custom Styling

Make your forms match your brand or aesthetic with custom styling options:

Color Presets

Choose from pre-made color schemes like Rose, Ocean, Forest, Sunset, and more

Custom Colors

Pick your own background, text, primary, and border colors

Border Radius

Choose from sharp, small, medium, large, or full rounded corners

Fonts

Select from Inter, Nunito, Poppins, and other font families

Sharing Forms

Once your form is ready, here's how to share it:

  1. Make sure your form is Published (toggle in Settings)
  2. Click the Share button in the form editor header
  3. Copy the public form link
  4. Share the link anywhere - Discord, Twitter, DMs, etc.
